Common causes

Varicocele – Enlarged veins in the scrotum can raise testicular temperature and affect sperm production. It is one of the common reversible causes of low sperm count.

Hormone imbalance – Problems with testosterone, pituitary hormones, thyroid hormones, or other hormone pathways can reduce sperm production.

Infections – Some infections can affect sperm production or block sperm transport.

Smoking and heavy alcohol use. – These can reduce sperm count, sperm movement, and overall sperm quality.

Obesity and poor lifestyle – Excess weight, poor sleep, stress, unhealthy diet, and lack of exercise can affect fertility.

Heat exposure – Frequent hot tubs, saunas, tight underwear, or prolonged heat near the testes may affect sperm production.

Medicines, steroids, or toxins – Anabolic steroids, some medicines, chemotherapy, radiation, pesticides, and chemicals may lower sperm count.

Blocked sperm transport or testicular problems – Blockages, undescended testis, injury, prior surgery, or testicular disease can reduce sperm output.

Management

Semen analysis and doctor evaluation – A semen test helps confirm sperm count, movement, and shape. Sometimes the test needs to be repeated because sperm count can vary.

Find and treat the cause – Treatment depends on whether the cause is varicocele, infection, hormone issue, blockage, medicine effect, or lifestyle-related.

Improve lifestyle – Stop smoking, limit alcohol, exercise regularly, sleep well, manage stress, and maintain a healthy weight.

Reduce heat and toxin exposure – Avoid frequent hot tubs, saunas, anabolic steroids, harmful chemicals, and unnecessary heat around the testes.

Review medicines with a doctor – Do not stop prescribed medicines on your own. Ask your doctor if any medicine may be affecting fertility.

Fertility treatment if needed – Some couples may need treatments like IUI, IVF, or ICSI depending on sperm count and other fertility factors.
